Kaya HR will reimburse you for a flu shot (up to $50) for yourself, your spouse/domestic partner, and your dependent children. All you need to do is get your flu shot (at any location convenient to you) and complete the form below, including uploading a receipt with all the required information.
Flu shots must be given between 9/15/2025 – 11/15/2025 and submission of the form is due no later than 11/20/2025. Contact Michelle (michelle@kayahr.com) with any questions!
